If you are applying for a regular SIG you can simply email this form to [email protected].

However, if you're applying for an extended SIG we need a physical copy; email or fax will not suffice. Please print out this agreement, fill in your name in the blanks as appropriate, sign and date it and mail it to:

Remember also that you must have signed and sent in the VCN Web Space Agreement if you are applying for an extended SIG. You only need to do this once. So if you're already got a SIG or personal Web page on the system there's no need to send in duplicate copies of the Web Space Agreement.
