Name of File Content and Purpose
User Accounts To Validate Registration Forms for users who are ready to be validated as first-time users. After validation, put into Forms To File.

If there are problems with a Registration Form, create a request in Request Tracker and contact the user to explain the problem and how to resolve it.

Write the date and the RT # on the top right corner of the Registration Form and put it in the Request Tracker Problems file folder.
Forms To File Registration Forms ready to be filed. Filing Cabinets are across the hall in the equipment room - file Individual's Registration Forms according to last name and Group Registration Forms in the separate drawer provided.
Request Tracker Problems Paper relating to problems logged in Request Tracker, such as Registration Forms completed incorrectly or submitted without an online registration completed. The RT# and date should be in the top right corner. Check this file when a user tells you they received a call from us about their registration.

Completed password reset forms should be placed on top of the green file folder in the red tray on the filing cabinet in the Equipment Room.
