SUBJECT:  New Form 100 Membership Document

Enclosed is a supply of the latest revision of the Form 100 Membership Document intended for use in your jurisdiction.  This form bears the revision date of either 9/11 or 10/11 following the form number and replaces all previous versions of the Form 100.  Older versions of the form are not to be used following receipt of this mailing.  Thus, please destroy any existing supplies of forms with older revision dates.
 
The major revision to the Form 100 is related to the New Member Annuity offer for the applicant and his wife.  The interest rate quoted on previous revisions has changed and is no longer printed on the reverse side of the Council Copy of the form.  However, the annuity is still available to new applicants and their wives and can provide an excellent opportunity for planning financial stability in retirement. The current New Member Annuity interest rate can be obtained by dialing 800-524-3611 and selecting option (7) when prompted.  The product remains available to new members and their spouses between the ages of 18 and 85.
 
A New Member Annuity Plan information pamphlet (Item #4794) is available from the Supply Department.  It is recommended that you order a supply of these pamphlets and provide one to each new applicant for membership.  You may order additional Form 100s, in reasonable quantities, from Supply as well.
