Some of the following forms are from, or adapted from, publication 1436, "Council Report Forms Booklet", copies of which are held by the Grand Knight and Financial Secretary of all councils. Others are British Columbia & Yukon State forms.

Each form may be displayed by selecting the form number in the "Form No." column below. It may then be printed, filled out and submitted to Supreme Council, the State Deputy and/or your District Deputy as indicated in the "Send to" column. Some forms may be completed on-screen prior to printing.

In some cases, the forms available here carry a former year's dates. Those dates should be corrected before submitting the completed forms. The current year's forms will be posted when available.

Please note that the following table has two parts. The first part lists forms that have a specific due date and are listed by date. The second part, "OTHER FORMS and PUBLICATIONS", lists forms that have no single due date; i.e. are submitted at various times throughout the year or as needed.
