Forms - Registration Package

A registration package consists of the following forms (broken down into the initial registration forms and then additional forms to be submitted after acceptance). These forms must be downloaded and printed. The forms and the deposit cheque (Class Fees and Times should be returned to a member of the enrolment team. You can also request a package from the enrolment team.

Initial Registration Package

• registration form -- 2’s class, 3’s class, 4’s class
• parent agreement form (2 copies) -- 2’s class, 3’s and 4’s class
• parent job list
• Don�t forget your non-refundable deposit cheque (registration fee and last month�s tuition)

Additional Forms to be Submitted after Acceptance

• interest information form
• certification of immunization
• childcare facilities registration form
• emergency consent form
• doctor’s letter (required for 3’s and 4’s class only)
